Community services navigator in Grafton
Sarah LaDuke, community services navigator for Ozaukee County, now has offices in the Sirona Recovery office at the Ozaukee Nonprofit Center in Grafton.
The program provides low-barrier, trauma-informed support to individuals and families navigating housing instability, mental health challenges, substance use and other life circumstances.
LaDuke’s work includes helping individuals identify needs and strengths, creating personalized action plans, connecting people with local resources and support services, offering advocacy and consistent follow-up and supporting long-term stability and independence.
The Community Services Navigator program, funded by United Way of Northern Ozaukee, is a collaborative effort between Advocates of Ozaukee, Ozaukee County Human Services, Lakeshore Regional Child Advocacy Center, and Sirona Recovery.
“Sarah plays a vital role in helping people connect with services that support safety, wellness and independence,” Barb Fischer, executive director of Advocates of Ozaukee, said. “This collaboration ensures care is coordinated across agencies. We’re grateful to Sirona Recovery for providing space that supports this important work.”
LaDuke’s hours at the office, 2360 Dakota Dr. in Grafton, are from 9 a.m. to 4 p.m. Mondays through Thursdays and 9 a.m. to noon Fridays.
